Scheduling an Appointment

For your convenience, our office provides same day appointments for routine and sick visits. To schedule an appointment, please call our office at 231-672-3177 during the following hours:

  • Monday - Thursday  7:30a - 5:00p
  • Friday  7:30a - 4:00p

Appointment Guidelines

  • Please notify us 24 hours in advance if you need to cancel your scheduled appointment.
  • Please arrive 10 minutes early for your appointment to complete any required administrative tasks.
  • If you are a new patient, having your previous medical records sent to our office prior to your visit will aid the physician with your treatment.
  • Please bring a complete list of medications that you or your child is taking or the actual bottles. Please also have a list of all vitamins, herbal supplements, or over the counter medication you or your child is taking, since this can affect the body and the reaction to the medication we may prescribe.
  • Verify your insurance's coverage of our services prior to your visit.
  • Bring your insurance cards with you every time you visit us for an appointment.
  • You are responsible for your co-pay at the time of service. The co-pay is set by your insurance company and will be collected before you see the physician.

 

 

Prescriptions and Refills

We want you to refill you prescriptions at the time of your doctor's visit. If you take more than one or two medicines, it is a good idea to bring all of your medicines in a bag with you to each visit. That way we can verify that your medicines are being taken correctly.

We have a 24 hour prescription refill line, where you can leave a message about needing medication refills.  These will be answered on the next business day. Prescriptions will not be refilled after hours or on weekends. Please note: for any phone call for a medication refill after 3pm on any weekday, we will not refill those medications until the following day.

 

Billing and Insurance Information

Unless we participate with a particular insurance carrier, medical services that are provided are expected to be paid for at the time of service. By upholding your responsibility for paying for services, we can hold the costs down and spend more time on medical care and not trying to collect debts due to us. We accept cash, personal checks, MasterCard and Visa credit cards as appropriate forms of payment.

Insurance payments vary with the type of policy and insurance carrier. Check with your carrier if you have questions regarding your coverage. However, the charges resulting from the delivery of medical care are your responsibility. If you have any questions, please refer to our credit policy which you signed when registering. If you have any problems paying your bill, tell us so we can assist you.

If you are covered by more than one insurance company, please let us know which company is primary (to be filed first and is responsible for payment first) and which is secondary (to be filed after the primary has paid). We cannot be responsible for knowing which insurance would be filed first.

Please advise our office of any name changes or change in your address, phone number, marital status, insurance information, etc. that have occurred since your last visit.

Any additional information requested by the insurance company to update their file is YOUR responsibility. If payment of a claim is held up because of information you need to supply to your carrier, you will need to contact them with the information requested and then contact our billing office to get the claim reprocessed. Accounts are routinely turned over to a collection agency when payment has not been paid within a reasonable amount of time.

After Hours/24 Hour Coverage

One of our physicians is available 24 hours a day, 7 days a week for urgent issues or questions. They can be reached by calling 737-5689 and the doctor on call will be paged. It may take a few minutes for them to return your call. Do not page the on call doctor for routine labs and x-ray results, as they will not have that information available to them. They will not refill medications.

Holidays

We are closed on New Years Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, Christmas Eve and Christmas Day. We are also closed on the afternoon of Good Friday, and New Years Eve.
